Commonly asked questions

This isn’t my first time there, do i need to fill out the registration forms again?

If this is not your first time meeting with our doctors for a comprehensive eye exam, you do not need to fill out the intake forms again. We may ask you a few questions just to update your records at your visit though!

i’ve been going to another optometrist for a long time. can i send my records over to you?

You sure can! You’ll just need to sign a “release of records” form so that we can get obtain that information from your last office.

do you do telehealth appointments?

Our doctors do not perform telehealth appointments at this time. We prefer to discuss your concerns in person, especially in the case that we need to check your vision.

do your doctors perform pediatric exams?

We love children and would love to see your whole family! Unfortunately, we only see children ages 6 or older at this time.

Can I get my Pupillary Distance measurements?

Due to liability purposes, we do not provide PD measurements for outside orders at this time. If you are purchasing online, please follow their instructions on how to measure your pupillary distance at home.
